A 2026 study in The BMJ followed roughly 50,000 adults with type 2 diabetes and found that those who started a GLP-1 medication — semaglutide, tirzepatide, the whole family — were 37% to 68% more likely to be diagnosed with hair loss than people on other diabetes drugs. Before you panic: the extra risk works out to about 2 to 3 people per 1,000 per year, and it was concentrated in non-scarring alopecia — the reversible kind of shedding that follows fast weight loss. Your follicles aren't dead. They're on strike. And strikes end.
You did the thing. You went on the medication, the appetite noise went quiet, the jeans fit, and for the first time in a while you felt like a person who had their life together.
And then you looked at your shower drain.
If you've been quietly Googling "Ozempic hair loss" at 1 a.m. while telling yourself it's probably the season, or hormones, or stress, or whatever — good news and bad news. The bad news: you weren't imagining it. The good news: it's now been studied properly, and the story is much less frightening than the TikToks.
Is GLP-1 hair loss actually real, or is it a TikTok thing?
Short answer: both — but now with receipts.
For a couple of years, GLP-1 hair loss lived entirely in the comments section. Anecdotes, before-and-afters, one very upsetting ponytail video. The FDA said it was "looking into it," which is government for we also saw the ponytail video.
Then in July 2026, researchers at the University of Pennsylvania published the first serious study on it in The BMJ, one of the most respected medical journals in the world [1]. They pulled real medical records from a large health system and tracked adults with type 2 diabetes who started a new medication between January 2019 and September 2024.
Roughly 12,000 people started a GLP-1. Another 26,000 started one of two other diabetes drug classes — the control groups, essentially. Anyone who already had a hair-loss diagnosis was excluded, so the study was hunting for new shedding only.
Then, using a method with the wonderfully dramatic name target trial emulation, they did statistical gymnastics to match the groups on age, weight, blood sugar, other conditions — dozens of factors — so the comparison would behave as much like a real clinical trial as observational data allows.
Translation: this is the most rigorous look we've had. Not a vibe. A study.
What did the BMJ study actually find?
The headline percentages are real. The context underneath them is what matters.

Two details matter more than those headline percentages, and they're the ones that got flattened in every news write-up.
One: the increase was concentrated in non-scarring alopecia — 53% and 72% higher, depending on the comparison group [1]. In derm terms, non-scarring is the category where the follicle is alive and well and has simply hit pause. Not the category where the follicle is gone. This is the single most reassuring sentence in the entire paper.
Two: the shedding clustered in the first year, peaking just before the 12-month mark. It has a beginning, a middle, and — crucially — an end.
Isn't a 68% increase enormous?
It sounds enormous because percentages are professional liars.
Here's the thing about a 68% increase: it depends entirely on what you're increasing from. Going from about 4 cases per 1,000 people to about 7 cases per 1,000 people is technically a 68% jump. It is also, in real human terms, two or three extra people out of a thousand, per year.
Which means more than 99% of people in this study did not get a hair-loss diagnosis in a given year — on either medication.
Now, a caveat in the other direction, because we're being honest here. This study counted people who went to a doctor and got a formal diagnosis coded into their chart. If you shed for four months, cried about it, bought a volumizing mousse and moved on — you're invisible to this data. So real-world shedding is almost certainly more common than 7 in 1,000. It's just usually milder than a doctor's-visit-level event.
Why would a weight-loss drug come for my hair?
Here's where it gets weirdly comforting: it's probably not the drug. It's the speed.
Your hair doesn't grow continuously, like a lawn. It grows in cycles. At any given moment, most of your follicles are actively growing (anagen) and a small minority are resting (telogen) [2]. This staggering is why you shed 50 to 100 hairs a day and never notice.
But when your body goes through something big — surgery, a high fever, childbirth, a brutal breakup, a crash diet, rapid weight loss — a whole cohort of follicles can shift into resting mode at the same time.
Then, about two to four months later, they all let go. Together. In the shower. On the same Tuesday.
This is called telogen effluvium, and it is the most theatrical, least dangerous form of hair loss there is [3]. It looks catastrophic precisely because it's synchronized. It's not that you're losing more hair overall — it's that months of normal shedding got compressed into a few dramatic weeks.
And GLP-1s? They cause fast weight loss and a genuinely large drop in how much you eat. Less food means less protein, less iron, less zinc, fewer B vitamins — the actual construction materials your follicles need [4]. Does GLP-1 hair loss grow back?
It grows back.

Am I likely to get it?
The study found the pattern held fairly consistently across age, sex, race, and which specific GLP-1 people took. But based on how this type of shedding works, some people are far more likely to notice it.
You're more at risk if you're:
- Losing weight very fast — the steeper the curve, the bigger the synchronized shed
- Eating dramatically less and not thinking about protein at all
- Already running low on iron, ferritin, vitamin D, or B12
- In perimenopause or menopause, where hormonal thinning is already on the guest list
- Carrying a family history of thinning hair
- Somewhere in the 3-to-12-month window after starting — peak shedding season

What should I actually do about it?
First and most important: do not quit your medication over your hair.
GLP-1s are treating something serious. Stopping abruptly has real consequences, and hair is not worth trading your metabolic health for. Talk to whoever prescribed it before changing anything.
Then work the fixable list.
1. Eat more protein. Genuinely more.
Hair is keratin. Keratin is protein. When appetite disappears, protein is the first casualty and nobody notices until their ponytail does [4]. Get a real protein source into every meal, even when you're not hungry. This is the highest-leverage item on this list and it costs nothing.
2. Get bloods done — and ask for ferritin specifically
Ferritin (iron stores), vitamin D, B12, zinc, thyroid. Low ferritin is one of the most common and most fixable causes of shedding, especially in women, and it hides behind "normal" iron results all the time [7]. Ask for the number, not the reassurance.
3. Ask your prescriber about slowing the pace
If the weight is falling off at a startling rate, your prescriber may be able to adjust the dose or the timeline. Slower is often kinder — to your hair and to the rest of you.
4. Take care of the scalp you've got
A scalp-focused shampoo and a targeted serum won't substitute for protein and iron. But keeping the scalp environment healthy while your body catches up is a real, sensible thing to do rather than nothing.
5. Take photos. Monthly.
